Containing all the basic information you need to be admitted to our hospital, this free card eliminates repetitive paperwork. Simply present your MED-KEY card along with your insurance card and another form of identification upon arrival at one of our admission stations.* The bar code on your card is encoded with your personal information, allowing you to avoid much of the required admission paperwork.

To receive your MED-KEY application form, call our Admitting Department at (601) 445-6210, or stop by the hospital and one will be made for you.

* In an effort to keep your records current, we ask that you advise us of any change in your name, address and insurance information. We will verify your personal information upon admission. It is still necessary to sign your name on the various forms required for a hospital procedure or admission.
